2017 Flu shots - Oct 16th

The Sanborn County Office of child and Family Services has set the date for our Annual Flu Shot Clinic - October 16th.  Forms were sent home with Pre-K thru 5th students tonight. 6th thru 8th students had forms placed on their lockers to be taken home.

Parents can access the forms by going to our web site - click on the Forms and Publications tab in the upper right corner, click on Google Drive, Health Folder, and Flu Shot Folder.

The Consent Form gives Marla permission to give the shot.  The Intake Form is to verify insurance - both must be completed and returned to the school office by October 5, 2017.
